A referral is requested before our intake team confirms your appointment with a fertility specialist at RCC.
Option 1: Your Own Doctor Ask your primary care physician – you can download a referral form and share with your physician. The completed form can be faxed to RCC or you can bring the completed form to your first appointment at RCC fertility
Option 2: Virtual Referral Book a same-day, no-fee, virtual referral appointment with our referral partner, Rocket Doctor.
